Below are the order and reorder forms for Commuter Check. We have a national order
form and three region-specific forms. The national form allows you to purchase Commuter
Checks, which are redeemable nationwide--with the exception of Philadelphia and
Chicago. The Philadelphia program is sponsored by DVRPC and the Chicago program
is sponsored by RTA. They each have unique vouchers specific to their region. The
New York form allows you to order both our national voucher and MetroCards, which
are used on the New York transit system.
